Churn Authorisation
If you are switching to Blue iQ from another provider you will need to download a churn authorisation form, which gives us your permission to move your broadband service over to Blue iQ. Click here to down load the form, then either fax it back to us on 07 4980 0001, mail it to PO Box 757, Emerald, QLD 4720 or drop it in to us at 5 Hogan's Road, Emerald, QLD 4720. If you have any problems opening the form please give us a call on 1300 Blue iQ.
